In all of our work, The Centre for MindBody Health (CMBH) is concerned about and respects the privacy of our clients. We appreciate the importance of ensuring a high degree of protection of personal information that you share with us. CMBH has established a Privacy Statement so that you understand what information we collect, how we use the information and the nature of our relationships with third parties who are given access to this information with your permission.

CMBH’s Privacy Statement applies to the collection, use and disclosure of personal information. This includes information that can be used to identify or contact a specific individual. It also encompasses personal health information such as medical records and other information about your health treatment.


You agree that we may collect, use and disclose your personal information in accordance with this Privacy Statement.

CMBH will collect, use and disclose your personal information without your consent in limited circumstances as permitted by law. For example, personal information may be used or disclosed without consent for the purpose of acting in respect of an emergency that threatens the life, health or security of an individual. In limited circumstances, we may be called upon to release your personal information in response to a court order, subpoena, search warrant, law or regulation. We will cooperate in responding to such requests, taking appropriate measures to ensure that the requester understands the sensitive nature of the personal information that they may receive.

Other than described above, we will not collect, use or disclose your personal information for purposes unrelated to the purposes described in this Privacy Statement without first obtaining your prior consent.

What personal information does CMBH collect?

CMBH does not collect personal information when you visit our web sites unless you choose to voluntarily provide such personal information. It is CMBH’s policy that personal information of the users of our site is private and confidential.

How is this personal information used?

CMBH will use personal information to provide our clients with information and services and to help us better understand your needs. Specifically, we use your information to communicate with you, to deliver services to you, to bill you for products/services you purchased, and to provide ongoing service and support.

Our Relationship to Third Parties

We have relationships with different third party service providers who help us provide services to you. We will not disclose your personal information to anyone other than our employees, our affiliates, our agents and those third parties that require such information in order to help us provide services to our clients and administer our business, including administering and maintaining our website. In order to facilitate our provision of services to you, your personal information may be used, stored and/or accessed by an affiliate or a third-party service provider outside of Canada. All such information will be protected in accordance with this Privacy Statement. However, your personal information would be subject to the laws of the country to which it is transferred and stored.

Our Security Measures

We are committed to protecting the security of your personal information. We have put in place physical, electronic, and managerial procedures to safeguard and help prevent unauthorized access, maintain data security, and correctly use your personal information and have clearly defined internal policies and practices. We apply security safeguards appropriate to the sensitivity of the information, such as retaining information in secure facilities and making personal information accessible only to authorized employees on a need-to-know basis.

Your personal information is stored on our database servers or hosted by third parties who have entered into agreements with us that require them to observe our Privacy Statement. We have implemented technological measures to prevent individuals from accessing personal information without authorization. Data centers are designed to be physically secure and protected from unauthorized access by unauthorized persons.

Although we will make reasonable efforts to protect personal information from loss, misuse, or alteration by third parties, you should be aware that there is always some risk that an unauthorized third party could find a way to thwart our security systems.


We will retain your personal information for the period of time that is required by law. When your personal information is no longer required, it will be destroyed or depersonalized.


CMBH will engage in periodic self-assessment to verify that it continues to be in compliance with this Privacy Statement.
